organization. A seasoned team of home care executives and a voluntary board of
directors over see the planning and delivery of care to thousands of families
throughout Northeast and Central Ohio. Our independent family of agencies, VNA
of Cleveland, VNA Mid-Ohio, Hospice and Palliative Care Partners of Ohio, VNA
CarePlus, and VNA HouseCalls, provide a full range of care to persons wherever
they live, work or play.
Founded in 1902, VNA embraces the challenge of healthcare with more than 700
dedicated professionals nationally recognized for technology and healthcare
services. We hold steadfast to our mission to deliver care with expertise and
compassion to promote health, independence and dignity.
The Visiting Nurse Association Healthcare Partners of Ohio teams up
with hospitals, physicians, community agencies, and insurance companies to
deliver high-quality, family centered care to a 17-county area in Northeast and
Central Ohio.

VNA Cleveland—Home Health
VNA Cleveland provides state of the art, high-quality home healthcare to
individuals and families in Northeast Ohio. VNA Cleveland provides intermittent
home healthcare as prescribed by physicians. Services include skilled nursing,
rehabilitation, mental health, and personal care services. In-home
tele-monitoring services are also available.
VNA Mid-Ohio
VNA Mid-Ohio (VNAMO) provides high-quality, compassionate home
healthcare to individuals and families in Central Ohio. VNAMO provides
intermittent home healthcare as prescribed by physicians. Services include
skilled nursing, rehabilitation, mental health, and personal care services.
In-home tele-monitoring services are also available.
Hospice and Palliative Care Partners of Ohio (HPCPO)—End-of-Life Care
HPCPO offers special support for those with terminal illnesses. Our continuum
of care includes hospice, pain management, symptom control services, and a
variety of support services. With over 20 years of hospice care experience,
seasoned professionals guide persons and families to select the level of care
that fits their needs.
VNA CarePlus—Private Duty
VNA CarePlus provides skilled nursing and personal care assistances by the
hour, by the shift, or around the clock. CarePlus professionals make everyday
living easier for persons of all ages. VNA CarePlus offers special programs to
restore and support the personal and social needs of our clients.
VNA HouseCalls—In-Home Physician Services
VNA HouseCalls delivers primary medical care and consultation in the home to
persons who are homebound or have difficulty getting to the doctor’s office.
Medical care is delivered by a team of VNA Physicians and Advanced Practice
Nurses who diagnose and treat persons in the home.
