VNA HouseCalls delivers primary medical care and consultation to persons who have difficulty getting to the doctor’s office due to a chronic, progressive disease, or from sudden, escalating symptoms of an illness.

Medical care is delivered by a team of VNA Physicians and Advanced Practice Nurses who bring the doctor’s office to wherever the person resides in Northeast Ohio. VNA HouseCalls provides primary medical care and/or consultation with patient’s primary or specialty care physician.

 
Assessment, diagnosis, and plan for treatment
Management of pain and distressing symptoms
Link and referrals to community resources
Ongoing medical care oversight and coordination
Patient and caregiver education
Routine diagnostic procedures and treatments
Access to off-hour support and consultation


 Persons who are essentially homebound
Persons for whom office based medical visits are
     a hard-ship due to:
